A two-stage framework for prioritizing the clinical novelty of disease trajectories found in longitudinal electronic health records

A Two-Level Clinical Novelty Prioritization Framework for Disease Trajectories Discovered from Longitudinal Electronic Health Records

Science & bioClassic ML, recsys & searchHealthcare & bio Advanced cs.LG

The authors took about 100,000 disease trajectories found through EHR graph analysis and ranked them in two stages, by distance from literature embeddings and by confidence, to pick out semantically novel trajectories.

Problem

Analyzing electronic health records (EHRs) as a graph yields a list of disease trajectories. But appearing often in the data does not mean a trajectory is medically new. A criterion is needed to decide which of the many trajectories to review first.

Method

The study used 99,527 branching trajectory motifs that had already been discovered and fixed in MIMIC-IV v3.1. It computed 'semantic sparsity' at the trajectory level and the node-to-endpoint level separately from a confidence-weighted 'clinical priority'. For each motif, three texts in a fixed format were created and embedded with BAAI/bge-small-en-v1.5. Novelty was then calibrated using the mean cosine distance to the top 20 nearest of 166,972 PubMed papers published before 2023. Robustness was analyzed by varying the number of neighbors, templates, encoders, scoring settings, PrimeKG, calibrated RND and more.

Results

Limitations

This analysis is based on the abstract. The authors state that the results neither discover causal relationships nor reflect novelty judgments by clinicians. The data come from a single institution (MIMIC-IV), and the top-k lists change with the settings. No improvement in discrimination performance was confirmed.

Why it matters

It proposes a way to choose which of the many automatically discovered disease trajectories researchers should look at first, based on novelty relative to the literature and on confidence. It also shows quantitatively how unstable those rankings can be.

Key terms

Disease Trajectory
The sequence of diagnoses that appear in a patient over time. Here it is represented as a branching motif in a graph.
Semantic Sparsity
A novelty measure based on how far a trajectory, converted to text and embedded, lies from the embeddings of existing literature.
MIMIC-IV
A widely used clinical database that releases de-identified records of intensive care and hospitalized patients from a single US medical institution.
